National Qualifier Reports

INSTRUCTIONS

1. Download Excel form HERE, complete it.  The first two sheets (Report Form and Registration) are the most important sheets.  Enter the relevant information (if you only had six players, only fill out six fields).  The other sheets are there if you wish to use them (you can scan the brackets and other paperwork if you so desire).  Save the file. (YOU CAN ALSO USE YOUR OWN FORMS, IF YOU WISH).
2. With this web form you can send all information of your ADO tournament (i.e. bracket sheets, photos, press release / tournament report etc).
3. Post all with THIS web form
4. After you have sent this form, you receive a copy of it in your email.

ADO executives (including Communications Officer, Chief Financial Officer and President) receive results via this web form.

    We prefer you use the attach ADO National Qualifier Results Excel, but it is not mandatory. You can use your own forms and upload them.
    You may attach up to 6 attachments i.e. press release and photos. Please write photos copyright information into additional information text field.
